Continuing Healthcare
NHS Continuing Healthcare (CHC), is a package of on-going care arranged and funded solely by the NHS, rather than Social Services. It’s dependent on the needs of the person; for some people this will be in their own home and for others in a care home.
Applying, and demonstrating the ‘primary health need’ of the person you are caring for can be a complex process, and it’s important to complete the form correctly to give the best chance of getting an award.
