Continuing Healthcare
NHS Continuing Heathcare (CHC), is a package of on-going care arranged and funded solely by the NHS, rather than Social Services. It’s dependent on the needs of the person; for some people this will be in their own home and for others in a care home.
Applying, and demonstrating the ‘primary health need’ of the person you are caring for can be a complex process, and it’s important to complete the form correctly to give the best chance of getting an award.
Expert Lesley Palmer will run through completing the form, giving hints and tips on how to present the facts in the best way to ensure they are looked at.
