Surrey Choices and SCC's Transition Team will be talking through supporting young people with disabilities as they transition towards adulthood.

Next Steps / Transition workshop

Find out how the Transition Team at Surrey County Council support young people from the age of 14 to 25 with disabilities and their families on their journey to adulthood. The presenter will be Lisa Woodruff Truscott, Carers Practice Advisor.

Surrey Choices will also be joining us to give a presentation on their offer, including employment and moving on after education. The presenter will be Louise Hayes, Senior Operations Manager.
