Referring an unpaid carer on to Action for Carers will ensure they get the help and support they need. Thank you.

If you are a GP, please use the Surrey Carer’s Prescription (GP version) to refer a carer.
If you are any other kind of health care professional, please use this version of the Surrey Carer’s Prescription.
All other professionals working with carers (schools, benefits advisers, social workers etc.) can use our referral form (where carers can also refer themselves).
