Referring an unpaid carer on to Action for Carers will ensure they get the help and support they need. Thank you.
- If you are a GP, please use the Surrey Carer’s Prescription (GP version) to refer a carer.
- If you are any other kind of health care professional, please use this version of the Surrey Carer’s Prescription.
- All other professionals working with carers (schools, benefits advisers, social workers etc.) can use our adult referral form, and young carer referral form.

Please note that if you are looking to get a particular service for a carer (such as support from our Moving and Handling team) that is an option within these forms.
Carers registering themselves
Please note that children above 13, parents of young carers, and adults can all register themselves.
