 Please note:  The Carers Prescription referral available through this website is for GP use only.  If you hold a different post in the NHS, such as School Nurse or District Nurse, please could you use the following link to make a Carers Prescription referral  www.surreynhscarersprescription.org.uk.

The process for a GP Carer Break referral:

If a patient meets the eligibility criteria outlined in the supporting material and you feel clinically they would benefit from this scheme, simply fill in the short referral form. This secure form will be sent to Surrey Independent Living Council (SILC) who are responsible for the administration.  You will receive an email back with a reference number confirming receipt. Once SILC has processed the form and arranged the payment to the Carer you will get an email confirmation. This can then be put onto the patient/ Carer records.

Changes to the GP Carer Breaks Service came into effect on 1 October 2016 - these are detailed in the Carers Prescription Guidance
